What is "Ozempic face" and can Mounjaro or Wegovy cause sagging skin? Learn why loose skin happens after weight loss, how to reduce it and what the options are.

What is "Ozempic face"?

"Ozempic face" is an informal term for the gaunt, hollow or aged facial appearance that some people notice after losing a significant amount of weight with medicines such as Ozempic, Wegovy or Mounjaro.

You may also see the terms "Mounjaro face", "Wegovy face" or even "Ozempic body".

There are three things worth knowing straight away:

  • It is not a medical diagnosis. It is a phrase popularised by social media and the press.
  • It is not caused directly by the medicine. It is the result of losing fat, including fat from the face.
  • It can happen with any significant weight loss, whether through diet, surgery or medication.

The same is true of loose or sagging skin elsewhere on the body.

This guide explains why these changes happen, who is most likely to notice them, what you can realistically do to reduce them, and what the options are if loose skin remains.

Why does weight loss change your face?

Your face contains pads of fat that sit beneath the skin and give it shape, fullness and a smooth contour.

When you lose a substantial amount of weight, you lose fat from all over your body. You cannot choose where it comes from, and that includes your face.

As facial fat reduces, you may notice:

  • Hollowing of the cheeks and temples
  • More prominent cheekbones and jawline
  • Deeper lines between the nose and mouth
  • Sunken areas under the eyes
  • Looser skin around the jaw and neck
  • More visible fine lines and wrinkles

For some people, this is a welcome change and makes their features look more defined.

For others, particularly those who lose weight quickly or are over 40, the loss of volume can make them look older or more tired than they feel.

Why does skin become loose after weight loss?

Skin is an elastic organ. It stretches as the body grows and can shrink back to some extent when the body gets smaller.

Its ability to bounce back depends on two proteins in the deeper layer of the skin:

  • Collagen, which provides structure and firmness
  • Elastin, which allows skin to stretch and recoil

When skin has been stretched for a long time, these fibres can become damaged. When the fat beneath is lost, the skin may not fully tighten again.

Common areas for loose skin after significant weight loss include:

  • The abdomen
  • Upper arms
  • Inner thighs
  • Breasts
  • Buttocks
  • Neck and jawline

Do Mounjaro and Wegovy cause sagging skin?

Not directly.

There is no good evidence that tirzepatide or semaglutide damage the skin, break down collagen or age the face.

The changes are a consequence of how much weight is lost and how quickly.

Because these medicines can produce greater weight loss than most people achieve with diet alone, often 15% to 20% of body weight on average in clinical trials, skin changes are simply more noticeable.

You can read about typical results in our article on how much weight you can lose on Mounjaro or Wegovy.

Who is most likely to notice it?

Several factors influence how your skin responds to weight loss:

Age

Collagen and elastin production decline naturally with age. People over 40 are generally more likely to notice loose skin and facial volume loss.

How much weight you lose

The more weight you lose, the more likely it is that some loose skin will remain.

How quickly you lose it

Rapid weight loss gives the skin less time to adapt.

How long you were overweight

Skin that has been stretched for many years is less likely to retract fully.

Genetics

Some people naturally have more elastic skin than others.

Sun exposure

Years of sun damage break down collagen and elastin.

Smoking

Smoking reduces blood flow to the skin and accelerates skin ageing.

Muscle loss

Losing muscle as well as fat can make the face and body look less firm and more hollow.

Can you prevent "Ozempic face" and loose skin?

You cannot completely prevent these changes if you lose a large amount of weight.

However, you can reduce their impact.

1. Lose weight at a steady pace

Slower weight loss gives your skin more time to adapt.

Do not rush dose increases, and do not try to speed things up by eating as little as possible. If you are losing weight very rapidly, tell your prescriber. Staying on your current dose for longer may be an option.

2. Protect your muscle

Muscle gives shape and support to the body and face.

Significant weight loss can involve some loss of lean tissue. Strength training and adequate protein help to preserve it, which improves how firm and toned your body looks as the fat comes off.

Read our article on weight loss injections and muscle loss.

3. Eat enough protein

Collagen is a protein, and your body needs amino acids to make and repair it.

Aim to include protein at every meal. Our guide on what to eat on Mounjaro or Wegovy explains how.

4. Eat a varied, nutrient-rich diet

Nutrients that support healthy skin include:

  • Vitamin C, needed to make collagen (found in citrus fruit, berries, peppers and broccoli)
  • Zinc (found in meat, shellfish, dairy, pulses and seeds)
  • Omega-3 fats (found in oily fish, walnuts and flaxseed)
  • Vitamin A and antioxidants from colourful vegetables and fruit

A balanced diet is preferable to high-dose supplements.

5. Stay hydrated

Dehydrated skin looks duller, drier and more lined. Reduced appetite often means reduced fluid intake, so drink regularly throughout the day.

6. Protect your skin from the sun

Ultraviolet light is the single biggest external cause of collagen breakdown.

Use a broad-spectrum sunscreen of at least SPF 30 on your face every day, and avoid sunbeds.

7. Do not smoke

Stopping smoking is one of the most effective things you can do for your skin, as well as your general health.

8. Look after your skin

A simple skincare routine can improve the appearance and feel of the skin:

  • A gentle cleanser
  • A daily moisturiser
  • Daily sunscreen
  • Products containing retinoids, which have good evidence for improving fine lines and skin texture (not suitable during pregnancy)

Skincare will not replace lost facial volume or remove loose skin, but it can improve skin quality.

Do collagen supplements work?

Collagen supplements are widely promoted for skin firmness.

Some studies suggest that hydrolysed collagen may produce modest improvements in skin hydration and elasticity. However, many studies are small, short-term or funded by manufacturers.

Collagen supplements will not prevent or reverse significant loose skin after major weight loss.

If you choose to try one, treat it as a possible small extra, not a solution. Getting enough total protein and vitamin C from your diet matters more.

Do firming creams and facial exercises work?

Firming creams can temporarily improve the appearance of the skin by hydrating it. They do not tighten significantly loose skin.

Facial exercises and facial massage are popular online. Evidence that they restore facial volume is very limited.

Neither is harmful, but expectations should be realistic.

Does loose skin improve over time?

Often, to some degree.

Skin can continue to retract for a year or more after your weight has stabilised.

Mild to moderate loose skin frequently improves, especially in younger people and those who build muscle.

More severe loose skin after very large weight loss is less likely to resolve fully by itself.

It is usually sensible to wait until your weight has been stable for at least several months before considering any procedures.

What are the treatment options for "Ozempic face"?

If you remain unhappy with changes to your face once your weight has stabilised, cosmetic treatments are available.

These include:

  • Dermal fillers, which can restore lost volume in areas such as the cheeks and temples
  • Collagen-stimulating injectable treatments
  • Skin-tightening treatments using radiofrequency, ultrasound or laser
  • Surgical procedures, such as a facelift, for more significant changes

These are cosmetic treatments and are not available on the NHS for this purpose.

If you are considering a cosmetic procedure:

  • Choose a qualified, registered healthcare professional, such as a doctor, nurse, dentist or pharmacist with appropriate training
  • Check they are on a recognised professional register
  • Ask what product is being used and what the risks are
  • Avoid practitioners who pressure you or offer time-limited deals
  • Tell them you are taking a weight loss injection

SynovaMed does not provide cosmetic procedures, and this article is not a recommendation to have one.

What are the options for loose skin on the body?

Non-surgical options

  • Strength training to build muscle under the skin
  • Supportive garments for comfort during exercise
  • Skin-tightening treatments, which may offer mild improvement

Surgery

For significant excess skin, body contouring surgery is the most effective option. Procedures include abdominoplasty (tummy tuck), arm lift, thigh lift and breast lift.

These are major operations with a recovery period and risks, and are usually considered only once weight has been stable for a sustained period.

Is skin removal surgery available on the NHS?

Body contouring surgery after weight loss is not routinely funded by the NHS.

In some circumstances, funding may be considered where excess skin is causing significant functional or medical problems, such as recurrent skin infections, but criteria are strict and vary by area. Your GP can advise.

Tell your surgical team about your injection

If you are having any operation or procedure involving general anaesthetic or deep sedation, you must tell the surgical and anaesthetic team that you are taking a weight loss injection. These medicines slow stomach emptying, which is relevant to anaesthetic safety.

Looking after loose skin

Loose skin can sometimes cause practical problems, particularly in skin folds.

To reduce irritation:

  • Keep skin folds clean and dry
  • Dry thoroughly after washing
  • Wear breathable fabrics
  • Use a barrier cream if rubbing is a problem

See a pharmacist or GP if you develop redness, soreness, a rash, broken skin or an unpleasant smell in a skin fold, as this may need treatment.

The emotional side of body changes

Changes in appearance during weight loss can be complicated.

Many people feel delighted with their progress. Others are surprised to find that their face looks older, or that loose skin makes them feel self-conscious, even after reaching a weight they have wanted for years.

Both reactions are completely normal.

It can help to remember:

  • Your face and body often continue to settle for many months after weight loss
  • Other people rarely notice the details you focus on in the mirror
  • The health benefits of losing excess weight, including for your heart, joints, blood sugar and sleep, are substantial
  • Feeling conflicted does not mean you made the wrong choice

If concerns about your appearance are affecting your mood, your eating or your daily life, please speak to your GP or your prescriber.

Should you stop treatment because of "Ozempic face"?

This is a personal decision, but it is worth weighing it carefully.

Stopping treatment may lead to weight regain, and regained weight does not necessarily return to the face in the same way.

Before stopping, speak to your prescriber. There may be other options, such as:

  • Slowing the pace of weight loss
  • Holding your dose rather than increasing
  • Moving to a maintenance plan once you reach a healthy weight
  • Adjusting your target weight, as a slightly higher weight may suit you better

Frequently Asked Questions

What is Ozempic face?

It is an informal term for the hollow, gaunt or aged facial appearance some people notice after significant weight loss with medicines such as Ozempic, Wegovy or Mounjaro.

Does Mounjaro cause "Mounjaro face"?

Not directly. Facial changes are caused by losing fat from the face, which can happen with any significant weight loss.

Does Wegovy cause sagging skin?

Wegovy does not damage the skin. Loose skin results from fat loss beneath skin that has been stretched and cannot fully retract.

How can I avoid Ozempic face?

You cannot fully prevent it, but steady weight loss, enough protein, strength training, good hydration, sun protection and not smoking can all help.

Does Ozempic face go away?

Facial appearance often improves somewhat once weight stabilises. Lost facial volume does not usually return fully unless weight is regained.

Will loose skin tighten after weight loss?

Mild to moderate loose skin often improves over a year or more. Significant excess skin after very large weight loss is less likely to resolve fully.

Do collagen supplements help loose skin?

Evidence suggests any benefit is modest at best. They will not reverse significant loose skin.

Can exercise tighten loose skin?

Exercise does not tighten skin itself, but building muscle can improve the shape and firmness of the area beneath it.

Can I have fillers while taking Mounjaro or Wegovy?

Cosmetic treatments can generally be carried out during treatment, but tell your practitioner about your medicine, and consider waiting until your weight has stabilised.

Is skin removal surgery available on the NHS after weight loss?

Not routinely. Funding is considered only in limited circumstances and criteria vary by area.

Is losing weight more slowly better for my skin?

Generally, yes. Steadier weight loss gives skin more time to adapt.

The bottom line: "Ozempic face" and loose skin

"Ozempic face" and loose skin are effects of significant weight loss, not direct effects of the medicine.

They can happen with any method of losing a large amount of weight.

You can reduce their impact if you:

Lose weight steadily.

Eat enough protein and a varied diet.

Do regular strength training.

Stay hydrated.

Protect your skin from the sun and do not smoke.

Give your body time to settle before considering any procedures.

For most people, the health benefits of reaching a healthier weight far outweigh these changes. But how you feel about your appearance matters too, and it is always worth raising your concerns with your prescriber.
