Getting sulphur burps, heartburn or acid reflux on Mounjaro or Wegovy? Learn why "egg burps" happen, the foods to avoid, remedies that help and when to get advice.

Do Mounjaro and Wegovy cause sulphur burps and acid reflux?

Yes. Burping, indigestion, heartburn and acid reflux are all recognised side effects of Mounjaro and Wegovy. Some people also experience unpleasant-smelling "sulphur burps", often described as tasting or smelling like rotten eggs.

These symptoms are rarely serious, but they can be uncomfortable and embarrassing.

The good news is that they are usually:

  • Most noticeable in the early weeks and after dose increases
  • Closely linked to what and how you eat
  • Manageable with some simple changes
  • Temporary for most people

This guide explains why these symptoms happen, which foods and habits make them worse, the remedies that can help, and the signs that mean you should seek medical advice.

What are sulphur burps?

All burps are caused by gas escaping from the stomach through the mouth.

Most burps are simply swallowed air and have no smell.

Sulphur burps contain hydrogen sulphide, a gas with a distinctive rotten-egg smell. It is produced when bacteria in the digestive system break down foods containing sulphur.

Sulphur burps can also be accompanied by:

  • Bloating
  • A feeling of fullness
  • Nausea
  • Wind with a similar smell
  • An unpleasant taste in the mouth

Why do weight loss injections cause them?

The main reason is slower digestion.

Mounjaro (tirzepatide) and Wegovy (semaglutide) slow the rate at which your stomach empties. This is part of how they help you feel full for longer.

But when food sits in the stomach and upper gut for longer:

  • Food has more time to ferment, producing more gas
  • Sulphur-containing foods are broken down for longer, releasing more hydrogen sulphide
  • Gas builds up and escapes upwards as burps

Other contributing factors include:

  • Large or fatty meals, which empty from the stomach even more slowly
  • Constipation, which slows the whole digestive system and increases gas
  • Fizzy drinks, which add gas directly
  • Eating quickly and swallowing air

Why do weight loss injections cause acid reflux?

Acid reflux happens when stomach contents flow back up into the oesophagus (food pipe).

It can cause:

  • Heartburn, a burning feeling in the chest
  • An acidic or bitter taste in the mouth
  • Regurgitation of food or fluid
  • A feeling of a lump in the throat
  • A cough or hoarse voice, especially in the morning

When the stomach stays full for longer, there is more pressure on the valve at the top of the stomach, making it easier for acid to escape upwards.

Reflux is therefore more likely:

  • After large meals
  • When lying down soon after eating
  • After fatty, spicy or acidic foods
  • After alcohol

Interestingly, in the longer term, losing excess weight often improves reflux, because there is less pressure on the stomach. Some people find their reflux is worse during the early months of treatment and then better than before once they have lost weight.

How common are these symptoms?

Burping, indigestion and reflux are listed in the patient information leaflets for both Mounjaro and Wegovy as common side effects, meaning they may affect up to 1 in 10 people.

They are most likely:

  • In the first few weeks of treatment
  • For a few days after each dose increase
  • In the day or two after your weekly injection

Most people find they ease as the body adapts.

Foods that can trigger sulphur burps

Sulphur is a normal and necessary part of the diet, and many sulphur-containing foods are very healthy. You do not need to avoid them permanently.

However, if sulphur burps are bothering you, it may help to temporarily reduce the foods most likely to contribute, then reintroduce them gradually.

High-sulphur foods

  • Eggs
  • Red meat and processed meats
  • Cruciferous vegetables: broccoli, cauliflower, cabbage, Brussels sprouts and kale
  • Onions, garlic and leeks
  • Dairy products, for some people
  • Beans and lentils
  • Dried fruit preserved with sulphites
  • Beer and wine

Other common triggers

  • Greasy, fried and very fatty foods, which stay in the stomach longest
  • Very large meals
  • Fizzy drinks, including sparkling water and diet drinks
  • Very sugary foods and sweeteners such as sorbitol
  • Alcohol

Everyone is different. Keeping a simple food and symptom diary for a week or two can help you identify your own triggers.

Foods and drinks that can trigger acid reflux

Common triggers for heartburn and reflux include:

  • Fatty and fried foods
  • Spicy foods
  • Tomatoes and tomato-based sauces
  • Citrus fruits and juices
  • Chocolate
  • Peppermint, in some people
  • Coffee and other caffeinated drinks
  • Alcohol
  • Fizzy drinks
  • Large meals and late-night eating

How to stop sulphur burps and reflux: eating habits

For many people, how they eat matters as much as what they eat.

Eat smaller meals

This is the single most effective change.

A large meal on top of slowed stomach emptying is the most common cause of both sulphur burps and reflux. Try smaller portions more often.

Eat slowly

Chew thoroughly and put your cutlery down between mouthfuls. Eating quickly means swallowing more air and overfilling the stomach before you notice you are full.

Stop when you are comfortably full

On these medicines, "full" arrives sooner than you are used to. Eating past that point often leads to discomfort a few hours later.

Avoid lying down after eating

Stay upright for at least two to three hours after a meal. Avoid eating a large meal late in the evening.

Limit fizzy drinks and drinking through straws

Both add gas to the stomach.

Reduce fatty and fried foods

These are the slowest foods to leave the stomach.

Stay hydrated

Sip water through the day. Good hydration supports digestion and helps prevent constipation.

Keep your bowels moving

Constipation and sulphur burps often go together. If you are constipated, treating it frequently improves burping and bloating too. See our guide to constipation on Mounjaro or Wegovy.

Go for a gentle walk after meals

Light movement helps food move through the digestive system.

For more on building meals that are easier to tolerate, read what to eat on Mounjaro or Wegovy.

Extra tips for night-time reflux

Reflux is often worse at night, when lying flat makes it easier for acid to travel upwards.

  • Finish eating at least three hours before bed
  • Raise the head of your bed by 10 to 20 cm, using blocks under the bed legs or a wedge under the mattress. Extra pillows alone are less effective.
  • Try sleeping on your left side
  • Avoid tight clothing around your waist
  • Limit alcohol and caffeine in the evening

Remedies that may help

Home remedies

  • Peppermint tea can help relieve wind and bloating, although it can worsen reflux in some people
  • Ginger tea may help with nausea and digestion
  • Chamomile or fennel tea are traditional options for bloating

These are unlikely to do harm, but evidence for them is limited.

Pharmacy treatments

Several over-the-counter treatments may help. Always ask a pharmacist which is suitable for you, particularly if you take other medicines or have other health conditions.

For wind and bloating:

  • Simeticone helps small gas bubbles combine so they are easier to pass

For heartburn and reflux:

  • Antacids neutralise stomach acid and give quick, short-term relief
  • Alginates (such as Gaviscon) form a protective layer on top of the stomach contents to help prevent reflux
  • Acid-reducing medicines, such as proton pump inhibitors (for example omeprazole or esomeprazole), reduce the amount of acid the stomach produces and are available from pharmacies for short-term use

For sulphur burps specifically:

  • Some people find products containing bismuth subsalicylate helpful. It is not suitable for everyone, including people who are allergic to aspirin, people taking certain medicines such as blood thinners, and during pregnancy or breastfeeding, so check with a pharmacist first.

Important: if you need indigestion or reflux remedies regularly for more than two weeks, speak to your GP or prescriber rather than continuing to self-treat.

Does the injection day or site make a difference?

Some people notice their symptoms are worst in the day or two after their injection, when levels of the medicine peak.

If so, it can help to:

  • Eat lighter, lower-fat meals for a couple of days after your injection
  • Avoid alcohol and rich food during this time
  • Plan your injection day so that this period falls at a convenient time

There is no reliable evidence that injecting in a different part of the body reduces digestive side effects, although some people report a personal preference. For technique and timing, see how to inject Mounjaro or Wegovy.

Should your dose be changed?

If symptoms are persistent or difficult to live with despite these measures, tell your prescriber.

Options may include:

  • Staying on your current dose for longer before increasing
  • Stepping down to a lower dose
  • Reviewing your diet and other medicines

Do not stop treatment or change your dose without advice. Our Mounjaro dosage guide and Wegovy dosage guide explain how doses can be adjusted.

What about alcohol?

Alcohol relaxes the valve at the top of the stomach, irritates the stomach lining and adds to dehydration. Beer, wine and fizzy mixers also add gas and sulphur compounds.

If burping or reflux is a problem, cutting back on alcohol is one of the quickest ways to see an improvement. Read more in our guide: Can you drink alcohol on Mounjaro or Wegovy?

Could something else be causing it?

Weight loss injections are a common cause of these symptoms, but not the only one.

Sulphur burps and reflux can also be caused by:

  • Stomach infections, including Helicobacter pylori and some gut infections
  • Other medicines, such as anti-inflammatory painkillers (for example ibuprofen)
  • Food intolerances
  • Gallbladder problems
  • Stomach ulcers
  • Hiatus hernia

If your symptoms began before you started treatment, are getting worse rather than better, or come with other symptoms, speak to your GP.

When to seek medical help

Seek urgent medical attention if you have:

  • Severe or persistent abdominal pain, particularly if it spreads to your back, with or without vomiting
  • Chest pain that is severe, spreads to your arm, neck or jaw, or comes with breathlessness or sweating. Call 999, as this may not be heartburn.
  • Vomiting blood, or vomit that looks like coffee grounds
  • Black, tarry stools
  • Persistent vomiting or being unable to keep fluids down
  • Yellowing of the skin or eyes

Speak to your GP or prescriber if you have:

  • Difficulty or pain when swallowing
  • Heartburn or indigestion most days for three weeks or more
  • Sulphur burps with persistent diarrhoea, fever or stomach cramps
  • Symptoms that do not improve with the measures above
  • Unintended weight loss beyond what is expected, or loss of appetite to the point of struggling to eat
