Feeling tired on Mounjaro or Wegovy? Learn why weight loss injections can cause fatigue, how long it lasts, how to boost your energy and when to see a doctor.
Do Mounjaro and Wegovy make you tired?
They can. Tiredness, or fatigue, is listed as a common side effect of both Mounjaro and Wegovy, meaning it may affect up to 1 in 10 people.
In most cases, the tiredness is:
- Mild to moderate
- Most noticeable in the first few weeks and after dose increases
- Linked to eating and drinking less, rather than to the medicine acting directly on your energy levels
- Temporary, improving as your body adapts
Many people actually report more energy once they have lost some weight and settled on treatment.
This guide explains the most likely reasons for feeling tired, what you can do to improve your energy, and how to recognise when tiredness needs to be checked by a doctor.
Why do weight loss injections cause tiredness?
There is rarely one single cause. Several factors usually overlap.
1. You are eating fewer calories
Mounjaro (tirzepatide) and Wegovy (semaglutide) reduce appetite, which is how they help you lose weight.
But food is your body's fuel. A sudden drop in energy intake, particularly in the early weeks, can leave you feeling flat, sluggish or weak until your body adapts.
If your intake falls very low, tiredness is one of the first signs.
2. You are not drinking enough
Dehydration is one of the most common and most overlooked causes of fatigue on weight loss injections.
When you eat less, you also take in less fluid from food. Many people feel less thirsty too. Nausea, vomiting or diarrhoea can add to fluid losses.
Even mild dehydration can cause tiredness, headaches, dizziness and poor concentration.
3. You are not eating enough protein or nutrients
When portions shrink, it is easy to fall short on protein, iron, B vitamins and other nutrients your body needs to produce energy.
Meals made up mostly of snacks or simple carbohydrates can also cause energy to rise and dip through the day.
4. Your blood sugar is changing
These medicines help regulate blood sugar. For most people without diabetes, this does not cause a problem.
However, long gaps between meals can leave some people feeling shaky, weak or tired.
If you have type 2 diabetes and take insulin or a sulfonylurea (such as gliclazide), combining these with a weight loss injection increases the risk of low blood sugar (hypoglycaemia). Symptoms include sweating, shakiness, hunger, confusion and tiredness. Your diabetes medicines may need adjusting, so speak to your prescriber or diabetes team.
5. Digestive side effects are wearing you down
Nausea, reflux, constipation or diarrhoea can disturb your sleep, reduce what you eat and simply be tiring to cope with.
6. Your sleep is disrupted
Night-time reflux, discomfort after a late meal or changes in routine can all affect sleep quality.
7. Your body is adapting to weight loss
Losing weight is a significant physical change. As your body adapts to using stored energy and a lower intake, it is common to feel more tired for a time.
8. You may be losing some muscle
Significant weight loss can include some loss of muscle as well as fat, particularly without enough protein or strength training. Less muscle can mean feeling weaker and tiring more easily.

When is tiredness most likely?
Tiredness tends to follow a pattern:
- The first two to four weeks of treatment
- The few days after each dose increase
- The day or two after your weekly injection, for some people
- Periods when you are eating or drinking very little
For most people, energy levels improve within a few weeks of settling on a dose.
How long does it last?
For many people, noticeable fatigue lasts a few days to a few weeks at a time, improving as the body adjusts to each dose.
If you have been feeling persistently tired for more than a few weeks on a stable dose, it is worth looking for another cause rather than assuming it is the injection.
How to boost your energy on Mounjaro or Wegovy
1. Eat regular meals, even when you are not hungry
Do not wait for hunger to tell you when to eat.
Aim for three smaller meals a day, with a snack or two if needed. Long gaps without food are a common cause of energy dips.
If your appetite is very low, set reminders to eat.
2. Prioritise protein
Protein helps maintain muscle, keeps you fuller for longer and supports steady energy.
Include a source at every meal: eggs, fish, chicken, lean meat, Greek yoghurt, cottage cheese, tofu, beans or lentils.
3. Include slow-release carbohydrates
Carbohydrates are your body's main source of energy. Cutting them out completely while already eating less can leave you drained.
Choose higher-fibre options that release energy gradually:
- Oats and porridge
- Wholegrain bread
- Brown rice and wholemeal pasta
- Potatoes with their skins
- Beans and lentils
- Fruit
4. Do not under-eat
Weight loss injections are designed to help you eat a sensible, reduced amount. They are not designed to help you eat as little as possible.
Very low intakes cause fatigue, dizziness, muscle loss, hair shedding and nutritional deficiencies.
Our guide on what to eat on Mounjaro or Wegovy includes practical meal ideas for a small appetite.
5. Drink more
Aim to drink regularly throughout the day, and check that your urine is a pale straw colour.
Water, milk, herbal teas, and tea and coffee in moderation all count. If you have had vomiting or diarrhoea, oral rehydration sachets from a pharmacy can help replace lost salts.
6. Keep moving
It may sound counterintuitive, but gentle activity usually improves energy.
A daily walk, some stretching or light strength exercises can lift mood and reduce fatigue. On days when you feel very tired, do less, but try not to stop altogether.
Avoid very intense exercise on days when you have eaten very little.
7. Protect your sleep
- Keep a regular bedtime and wake time
- Avoid large meals, alcohol and caffeine in the evening
- Finish eating two to three hours before bed, especially if you get reflux
- Keep your bedroom cool, dark and quiet
- Limit screens in the hour before bed
8. Go easy on alcohol
Alcohol disrupts sleep, dehydrates you and adds to tiredness the next day. Read more in our guide: Can you drink alcohol on Mounjaro or Wegovy?
9. Do not rely on caffeine
A cup of tea or coffee is fine, but too much caffeine can worsen reflux, disturb sleep and leave you more tired overall.
10. Time your injection to suit you
If you tend to feel tired the day after your injection, consider choosing an injection day that allows you to rest the following day. You can change your injection day as long as there are at least three days between doses. See missed a dose of Mounjaro or Wegovy? for the rules.
Could a vitamin deficiency be the cause?
Eating much less for many months can make it harder to get all the nutrients you need.
Deficiencies that commonly cause tiredness include:
- Iron, particularly in women who have periods
- Vitamin B12, particularly in people who eat little or no meat, fish or dairy, or who take metformin
- Folate
- Vitamin D, which everyone in the UK is advised to consider supplementing in autumn and winter
A standard multivitamin may be reasonable if your intake is low, but it is not a substitute for a balanced diet.
If you are persistently tired, ask your GP about blood tests rather than guessing. Do not take iron supplements without a confirmed deficiency, as too much iron can be harmful and iron tablets often worsen constipation.
Tiredness, dizziness and headaches
Tiredness often comes together with dizziness or headaches, particularly early in treatment.
Common causes are dehydration, not eating enough and, in people on diabetes or blood pressure medicines, low blood sugar or low blood pressure.
As you lose weight, your blood pressure may fall. If you take blood pressure tablets and start to feel light-headed, especially on standing up, tell your GP or prescriber. Your tablets may need reviewing.
Driving and operating machinery: if you feel dizzy, drowsy or weak, do not drive until you feel well. If you take insulin or a sulfonylurea, be particularly aware of the risk of low blood sugar while driving.
Could something else be causing your tiredness?
Tiredness is one of the most common symptoms seen by GPs, and it has many possible causes unrelated to weight loss injections.
These include:
- Anaemia
- Underactive thyroid
- Diabetes or changes in blood sugar control
- Sleep apnoea, which is more common in people with excess weight
- Depression, anxiety or stress
- Perimenopause and menopause
- Infections and post-viral fatigue
- Other medicines, such as some antihistamines, antidepressants and blood pressure tablets
- Poor sleep for any reason
If your tiredness started before you began treatment, has lasted more than a few weeks or is getting worse, do not assume the injection is responsible.
If hormonal changes may be relevant, see our guide to weight loss injections, menopause and HRT.
Tiredness and mood
Feeling physically drained can affect your mood, and low mood can make you feel more tired.
Some people also find that their relationship with food changes during treatment in ways they did not expect, which can feel unsettling at first.
If you notice persistent low mood, loss of interest in things you normally enjoy, anxiety or any thoughts of harming yourself, please speak to your GP promptly and let your prescriber know. If you feel you cannot keep yourself safe, call 999 or go to A&E. Support is available, and these feelings should always be taken seriously.
Should your dose be reviewed?
If tiredness is significant, tell your prescriber.
It may help to:
- Stay on your current dose for longer before increasing
- Step down to a lower dose
- Slow the rate of weight loss
Rapid weight loss is more likely to cause fatigue. Our article on how much weight you can lose on Mounjaro or Wegovy describes a realistic and sustainable pace.
Do not stop or change your dose without advice.
When to seek medical help
Seek urgent medical attention if you have:
- Severe weakness, confusion or fainting
- Chest pain, palpitations or severe breathlessness

- Symptoms of low blood sugar that do not improve after eating or drinking something sugary
- Yellowing of the skin or eyes
Speak to your GP or prescriber if:
- Tiredness lasts more than a few weeks
- It is getting worse rather than better
- It is affecting your work, driving or daily life
- You feel breathless, cold or look pale
- You have heavy periods
- You have persistent low mood
- You are struggling to eat or drink enough
